Slack Integration
Get instant notifications in Slack when someone submits a form, votes on a poll, or leaves feedback on a widget.

Setup
Go to Integrations
Navigate to Dashboard → Settings → Integrations and find the Slack card in the Notifications section.
Connect your workspace
Click Connect on the Slack card. You will be redirected to Slack to authorize FavForm.
Select a channel
Choose which Slack channel should receive notifications. You can select any public channel or private channel that the FavForm bot has been invited to.
Start receiving notifications
Once connected, all new submissions across your forms, polls, and widgets will automatically post to your selected Slack channel.
Notification format
Each notification includes:
- Form submissions: Form title, all field answers (up to 10), and response ID
- Poll votes: Poll title and the selected option
- Widget feedback: Widget title, star rating, and comment text
Disconnecting
To stop receiving notifications, go to Dashboard → Settings → Integrations and click Disconnect on the Slack card. You can reconnect at any time.
Troubleshooting
Not receiving notifications?
Make sure the FavForm bot has been added to your selected channel. For private channels, you need to manually invite the bot using /invite @FavForm
Connection failed?
Try disconnecting and reconnecting. If the issue persists, check that your Slack workspace allows third-party app installations.
